1.    Simply go back to the home page and click on the orange tab labeled “TELEHEALTH  APPT”.
2. Then, choose your desired appt time.
3. Provide info about your symptoms, medications, and insurance.
4. Once this info has been received, an intake packet will be sent for you to complete and return 24 H prior to your selected appt time.
5. Your appt with our provider will be confirmed, once eligibility is established.
